Re:         2005 Member Agency Funding Application

Attached is your 2005 Funding Application Form detailing all information you must submit to complete your 2005 UWSLA Member Agency Application Packet. The documents are also available on line at www.uwsla.com or via email upon request.

Please call the United Way office to acknowledge receipt of your application packet at 985-879-2461.

Your agency must provide one complete original application with all required attachments. 7 copies of all material are required to complete the packet, unless otherwise specified in the application.

There are changes to the application form again this again year.  Please pay close attention to the instructions regarding the new and additional information requested. It will be impossible to substitute the design of a previous year’s application. If any requirement seems confusing, or if you are not sure of precisely what we are asking you to report, please do not hesitate to call me at the United Way office for clarification. If any portion is omitted or deemed incomplete, the application will be denied.

The deadline for submission is Tuesday, September 28, 2004, by 2:00 pm.  If your packet is not received in our office by this deadline, your agency cannot be considered for 2005 funding.
